Essentially Revit as a program is wholly dynamic whereas CAD just has the option in a limited (though useful) scope. You are right that most people use Revit (or archiCAD, which seems like it might be on the way out) over 3d CAD I've only met one person in my career who liked 3d autocad but everyone else and myself find that it's dogshit and not worth learning. Revit's capability in that respect is quite different, but similar to some simpler dynamic blocks there are grips to resize some items very easily. Instead of dynamic blocks in Revit, it's more like a bunch of "families" of parts, and you pick the size of the part you want in a menu before/after placing it (easier if you do it before), and the utility of Revit just knows what should happen when you intersect a door with a wall (it just puts a void in the wall up to the frame of the door).

I'm not sure how they'd work in 3D CAD but once you learn how to make them in 2D it wouldn't be hard to figure it out for 3D (if it can do that, someone else please weigh in here).
